How to write a proper Warning letter?

I am working as a Manager in a private company and one of the employee who is under me is breaking the company rules, so I want to warn that person by issuing a warning letter but I am kinda not sure how to write the warning letter, so please help me to write a warning letter in minimum words.?

TIPS TO WRITE A WARNING LETTER:

-> A proper reason need to be stated for the discipline which will be like failure on not maintaining regular attendance, poor performance, inappropriate conduct and so on. Before starting of the letter you need to state the reason with proper proof and then continue on with the body of the letter.

-> Provide the facts with strong examples, dates and witnesses which will give the proof of why you are writing the warning letter.

-> Explain things which you have discussed with your colleague before of the things which you don't want him/her to do and it was done again and again.

-> Mention about the future consequences which he/she can face if the same things continued.

-> Finally add the signature line for the employee to sign just to have a understanding that he/she received the letter.

Now one important thing which you need to keep in mind before start framing the warning letter, if the broken rules is extremely mandatory then writing a warning letter will be ok but if it's not that critical then the better thing to do as a Manager is to call the employee and warn him/her verbally first to give a chance and if the same continued you can very well send in the warning letter.
